Submitting a notice

- Your name, mailing address, telephone number and email address.
- A statement of good-faith belief that the use is not authorized by the owner, its agent or the law.
- A statement, under penalty of perjury, that the notice is accurate and you are authorized to act.
- Your physical or electronic signature.
Counter-notice
If material is removed because of a mistake or misidentification, the affected party may submit a legally sufficient counter-notice containing the required identification, statements, jurisdiction consent and signature under applicable law.
Repeat infringement
Where appropriate and technically applicable, access may be restricted for users who repeatedly infringe intellectual-property rights.
